With two 3-way on/on coil splitting switches.

Neck: 9K  (splitting coil 4.5K)
Bridge: 15K (splitting coil 7.5K)
Four conductor wires, 
The black wire is hot, the green wire is ground, red and white is for splitting the coil
Pickguard: 3-ply PVC, white

As with any pickguard, some modification may be necessary for exact hole alignment, as there are many different hole patterns.
